Scope and content:

The collection consists of official forms of the Confederate States of America. Forms included: Letters patent; Tax in kind, form no. 2; Invoice of medicines, instruments, hospital stores, bedding, etc.; and miscellaneous Confederate hospital forms (notifications of death, applications for leave of absence, and medical certificates). The collection also consists of blank passes from the War Department (Ladies' passport, Fredericksburg Rail Road only, Central Rail Road only, Petersburg Rail Road only, and Soldier's passport).
